Motivational Interviewing

A recent addition to the therapeutic techniques appropriate to the hospital setting is motivational interviewing. In contrast to CBT and DBT, this modality is especially appropriate for the physician and may facilitate progress when a patient’s desire and willingness to move forward are absent or have stalled. The fundamental principle of motivational interviewing is to allow patients to identify their own priorities and goals and then to help them explore the obstacles they have faced in meeting them. The focus must be kept on the patient’s internal issues rather than outside factors, with gentle guidance toward an understanding of what changes need to occur. Although most popular in the hospital for patients not yet willing to seek substance use treatment, motivational interviewing is equally effective with other issues, such as medication adherence or participation in psychotherapy. Key elements are to approach the session with patience and an open mind, allow the patient time to reflect, and wait for the patient to come to the realization of the need for change and the direction it should take.
